healthwise

Healthwise is a team of caring professionals from Calvary Church including:
physicians, nurses, pharmacists, dentists, occupational therapists, physical therapists, respiratory therapists, social workers, health educators, and dieticians.

Services Healthwise Provides:

  • Education: The Healthwise Team will make health/wellness programs available.
  • Health Screenings: Screenings such as blood pressure, diabetes, etc., are periodically offered.
  • Counseling/Support: Upon request, a nurse is available to visit those who attend Calvary Church. This nurse can act as a support or referral person but will not administer medications or provide direct care. He/she will help you find the care you need.
  • Referrals: The Healthwise Team members serve as referral sources for health information and available resources.
  • Advocacy: A team member can assist you in understanding your care or medical terms, which are not familiar to you. He/she can also be your personal advocate in the health system.
  • The Healthwise Team does not attempt to duplicate services already in existence in the community. They do not provide hands on care or transportation and they will not diagnose or treat.

When to Call Healthwise:

  • When you or someone you know has been diagnosed with a serious illness and you have questions or concerns.
  • When you are struggling with a major life health transition such as deciding about nursing home care.
  • When you face early discharge from a hospital stay and need additional support services or have questions about community resources.
  • When you are struggling with chronic illness and need to locate a support group.
  • When you are caring for someone you love at home and need information, referrals, or support.
  • When you want to talk about any health concern, large or small, with a caring professional.
  • When you do not understand medical terminology and need simple explanations.
  • When you need an advocate to help you navigate the healthcare system.

Help! My Parents Are Aging! - Heathwise Team's Annual Spring Breakfast

         Tickets are $5 and will be on sale March 30, April 2, April 6 in the church atrium

         Are you part of the sandwich generation, trying to help your parents make the right choices? Or maybe you are facing having to make difficult decisions for yourself? It can be overwhelming. Our speakers will be Gretchen Schlicher and Anne Sheridan - social workers that specialize in helping the aging develop a long-term care plan. They'll help with decisions like whether it's best to stay in your home with help or if it's time to decide on a nursing facility. We'll also be learning what local resources can assist you in staying in your home longer, and what questions to ask when you're looking for a facility. Find out if you need long-term care insurance, a power of attorney and/or an advance directive. Find out the difference between independent living, assisted living, and skilled nursing care.
